Field with Yes or No

Hello,

I would like to create a new field . e.g. 'Remount' ( Y/N) . If 'Y' allow entry of  'Date' and ' Manufacturer'.   

Remount

Yes/No

Date:_______

Manufacturer: ___________

How do I do this?
