All questions and answers is not printing in Answer report for Vendor request in Ax 2012 R3

Hello Expertise,

I have created new vendor request from EP , fill up the vendor information , and questionnaire information for all question then  submit the workflow .Now able to see All questions , answer in answer Form like below.

But When generating Answer report for this only 2 questions and answer printing in Report.

I have other environment, there All Questions and answer printing in one page.

So is there any Setup or configuration or anything else ?

Please Suggest.
